Making Mid-year Changes

Once you enroll in the spending accounts for the calendar year, you may only make changes during the year if you have a qualified change in status. Any change you make to your participation must be directly related to the qualified change in status and must be requested within 30 days of the event. For example, if your spouse starts working full-time and you need day care, you may enroll in the Dependent Day Care FSA.

If you have one of the following qualifying changes in status, you may make a mid-year change:

To make a mid-year change:

  • Click here to obtain a "FSA Request for Change Form," or call Benefits Administration at 213-241-4262 and request this form.
  • Complete the form, attach any required documents and send it to Benefits Administration, P.O. Box 513307, Los Angeles, CA 90051-1307.
  • Indicate on the form which benefits or FSA contribution you want to change.
  • Once your change in status has been approved, your new FSA contribution will be reflected on your paycheck.
