Vision Plans

The District offers a choice of two vision plans.

WHO MAY ENROLL

EyeMed Vision Care

Eligible employees, U.S.-based retirees, COBRA, and AB528 participants

VSP

Eligible employees, Retirees, COBRA, and AB528 participants residing in California

Both vision plans provide some of the same benefits; however, there are some key differences, for example, with deductibles, non-network benefits, and locations.

Two-Year Enrollment Commitment: When choosing a vision plan, remember that the District requires you to remain enrolled in the plan you choose for two full plan years. For example, if you switched from EyeMed Vision Care to VSP for the 2007 plan year, you must remain in VSP for 2008.

Obtaining Vision Services

· To obtain vision services under EyeMed Vision Care, call 1-866-723-0514 or visit the web at www.eyemedvisioncare.com to find the provider nearest you. Network locations include; Pearle Vision, Sears Optical, Target Optical, Lens Crafters and other independent locations. You may also use an out-of-network provider; however the benefits you receive will be significantly reduced.

· To obtain vision services under VSP, make an appointment with a VSP Select Network provider. Call 1-800-877-7195 to obtain a listing of local VSP Select Network providers or visit the VSP website at www.vsp.com. You may also use an out-of-network provider for routine care, but your coverage may be reduced.
