Apply for Access to ISIS/LAUSDMAX
The EZ Access Request System is the new tool for school employees to request access to LAUSDMAX. This electronic method is faster and easier for both users applying for user roles as well as administrators authorizing requests. This application is available via the LAUSD network and from home over the Internet.

*Special Circumstances The paper requests (right column) must still be submitted under the following circumstances—
If you are the only administrator at your school
If you are employed by a local district/central office
If you are applying for the secondary user role of "School Office - Attendance Plus"
All Teachers Must Apply for Access to LAUSDMAX
EVERY SINGLE TEACHER must submit a separate access request for ISIS, in order to take attendance. Access is determined by the user role an individual is assigned. Please refer to the Guide to LAUSDMAX User Roles for a description of each user role.
Employees assigned to multiple schools must submit separate access requests for each school to which they are assigned. Please refer to the Quick Guide: Using EZ Access to Apply for LAUSDMAX User Roles for instructions on applying at multiple sites.
Administrators' Guide to Approving Access to LAUSDMAX
This guide provides step-by-step instructions to school administrators (principals and APs) for authorizing staff requests for LAUSDMAX access. Before, administrators would sign each paper request submitted by staff members. However, the EZ Access system replaces all paper requests, except in special circumstances*.

Fax Form: Removing ISIS Access from Former Employees
Submit this form for speedy removal of individual access to LAUSDMAX at your school. For security reasons, principals must immediately remove access from former employees.
ISIS User ID Form - Secondary Schools
Use this form only if a) you are the only administrator at your school, or b) you are applying for the "School Office - Attendance Plus" user role.
Addendum for 'Attendance Plus' User Role (Secondary Schools)
This form is required for the School Office - Attendance Plus user role; users must carefully read, sign, and submit this document with the ISIS User ID form for Secondary Schools.
Do not, under any circumstances, allow anybody to use your LAUSDMAX username and password.
Doing so would breach the security of the system, and violate District, State & Federal confidentiality laws. Password sharing is a punishable offense.
