Reporting Fraud, Waste, and Abuse
The OIG operates a Fraud Hotline so anyone can easily report allegations of fraud, waste and abuse occurring within the District. The OIG receives thousands of reports from concerned employees, vendors, and the public, most of which are either investigated by the OIG or referred to local district administrators for investigation and disposition.

Complaints regarding personnel matters and/or management are typically not investigated by the OIG unless there is evidence of wrongdoing. To ensure that your report is given adequate attention, you must provide reliable, accurate, and detailed information.

Prior to submitting a report, make sure you have the correct spelling of names, employee and vendor numbers (if available), position and program titles, work location, funding source, and all other pertinent detail. This will ensure that your report is handled quickly. Choose one of the three easy methods to report fraud, waste, and abuse:

  1. Complete the on-line Fraud Reporting Form and submit it electronically
  2. Or, print and complete the Fraud Reporting Form and mail it to the Office of the Inspector General;
  3. Or, call the Fraud Hotline (213-241-7778, toll-free 866-528-7364) and speak to an Investigator or leave a message.
